Eligibility
The eligibility criteria for services include:
A physical disability, which is defined as a physical impairment which prevents an individual from carrying out activities of daily living:
- unassisted
- or without endangering him/herself
- or within a conventional time period
- the Individual must require physical assistance with activities of living because his/her disability is likely to be permanent.
The Individual must be able to live in his/her own home with assistance from an attendant on an on-call basis and be able to be safely and responsibly left alone when physical assistance is not being provided.
The Individual must be capable of directing his/her own services. This includes determining what assistance he/she requires, when it is required, how it should be provided, and communicating such to those providing the assistance and cooperating with its provision.
The Individual must have his/her needs for professional health care services met within the existing community health care system.
The Individual must be unable to have his/her needs met adequately by the other home support or other services because of the intensity, irregularity, or unpredictability of his/her requirements over a 24-hour period.
The Individual must be able to pay his/her rent and meet the conventional costs of living.
The Individual must be able to understand, sign, and abide by the conditions contained within the Service Contract.
The Individual must be able to understand, sign, and abide by the terms of the tenant's lease.
